National 4-H Hall of Fame
Wisconsin 4-H has an annual opportunity to nominate an individual to The National 4-H Hall of Fame sponsored by NAE4-HYDP, in cooperation with the National 4-H Council and 4-H National Headquarters.
The National 4-H Hall of Fame honors 4-H volunteers, Extension staff and retirees, donors, and others who have made a significant impact on the 4-H program and/or 4-H members through the contribution of time, energy, financial resources, etc. Each state can submit one nominee for consideration at the National level.

Selection Criteria
Please include examples of the following in the nomination:
- Citizenship: participation in civic, community, state, regional, and national public service-related activities. Serving others in the community, especially 4-H members and volunteers.
- Leadership: qualities of leadership and acceptance of leadership responsibilities in civic, community, professional and business-related activities that benefit 4-H.
- Career Accomplishments: accomplishments and success in his/her chosen career or volunteer service as a mentor or role model.
- Character: showing the influence of 4-H involvement (if a 4-H alumni or volunteer) in the nominee’s life and/or his or her commitment to 4-H.
- Other beneficial information that would be helpful to the Selection Committee.

- Responses to the Essay Questions (see each question for character limits) & Short Biography of Nominee
Application Guidelines
- Application materials must be submitted by a nominator to the nominee’s State 4-H Office. Each State 4-H Office will select one nominee from its pool of nominations to be submitted for the national award.
- Packets missing any of the components will be eliminated (application form, essay responses).
