4-H Youth Health Form 2023-24

This form is REQUIRED for youth participating in overnight 4-H programs and/or activities. This is the same information that is collected through the Health Form in 4-H Online.

The fillable Camp Health form includes a full vaccination history.

The Health Form Addendum should be used with the regular health form for camps. It offers a vaccination history and extra lines for additional medications.
